A caregiver did not know the number of children. The caregiver stated there were 18 children in care and a total of 21 children were present in the classroom.

Based upon the information obtained from the DFPS Investigation, there was sufficient evidence to support a child was grabbed while in care.

A caregiver was observed alone with children without having an eligible background check.

An infant was observed swaddled in the crib during the inspection.

Four out of ten employee files reviewed were missing a high school diploma or equivalent.

In the infant B classroom, multiple bottles/sippy cups were not labeled with the children's names. Note: Corrected when the bottles were labeled by the caregiver.

Three caregivers who did not have background checks were present at the operation and had access to children in care in the infant A, school age, and Pre-K classrooms.

Two transportation vehicles used to transport children had first aid kits that were missing the following items: Vehicle 1- missing scissors, tweezers, thermometer, tape and gauze. Vehicle 2- missing thermometer and tweezers. Note: Corrected when the missing items were added to the first aid kits.

In the Infant A and Infant B classrooms, the diaper changing supplies were in an unlocked cabinet accessible to children.

A child with a known food allergy does not have a plan on file for review.

During this investigation, it was determined the operation allowed a child to return to school without a doctor's note after being notified by the child's parent that the child had two communicable diseases (pink eye and hand, foot, and mouth) during different time frames. Note: The child is no longer enrolled at the operation. The de-enrollment date was on 11/28/2022.

During this investigation, it was determined the operation did not notify parents of children enrolled about two communicable diseases within 48 hours of being made aware. The child's parent informed the operation that their child had pink eye (conjunctivitis) and hands, foot, and mouth disease. Note: The Director sent a notification to parents of children in the classroom where the child with the communicable diseases was present.

There is no documentation lock down drills are practiced at least four times a year.

The operation does not have a current employee list posted as required. Note: The Director created a current staff list and posted it during the inspection.

The operation has not certified annually the children's product safety website to view any recalled items. Note: The Director reviewed the list and certified by using form 2885 during the inspection.

A caregiver in the infant room (2m to 13m) did not state the correct number of children present. The caregiver stated there were 8 children but 9 children were present in the classroom,

The sink children use for hand washing and a toilet in the school age classroom needs repair. The sink is stopped up and is overflowing and 1 of 2 toilets are out of order.

Based on the information obtained, there is sufficient evidence to suggest that a caregiver administered prohibited discipline to a child in care. As of completion of the investigation the employee was immediately terminated.
